The first transnational conference “Making the case for integrated community care” took place in Hamburg, Germany, on 24-26 September 2018. It aimed to:

• develop a common understanding of the meaning of integrated community care;
• demonstrate through evidence the positive impact that integrated community care can have on improving care experiences, tackling inequalities, promoting care outcomes as well as cost-effectiveness;
• illustrate how integrated community care has been adopted in practice through established case examples as well as newly developed ‘promising practices’, including facilitated site visits to local innovations; and
• explore the essential design elements in the provision of integrated community care that make the approach successful. This included a focus on care delivery models and core organisational issues such as management, funding, governance, information sharing and accountability.
